- ✪ ✪ ✪ ✪ ✪While I am sue that Dr. Ketoff's medical procedures are fine he is deficient in his clerical process or his instructions to his staff. Apparently his staff is advised to fax the paperwork for a procedure to the appropriate hospital and also include the consent form. Unfortunately the hospital apparently requires an original consent form. No other physician is required to provide the ORIGINAL. I asked if I could have the paperwork faxed to me and I would sign the consent form in the presence of the hospital, thus making it an original. Unfortunately his staff does not have permission to do this. Utterly ridiculous. Hope his medical skills are more updated. Regards, Judith ForrestApril 27, 2017by Judith Forrest
